Membership Terms

Memberships are not insurance

All IV memberships offered by Family Clinic OKC IVs are elective, self-pay wellness programs. They are not health insurance, not a health plan, not a discount health plan, and not a substitute for medical coverage. Membership fees are not premiums and are not submitted to SoonerCare or any other payer.

Pricing

Monthly IV Membership: $197 per month plus a one-time $50 enrollment fee, including 2 IV treatments per month.

Quarterly IV Membership: $500 every three months with the enrollment fee waived, including 6 IV treatments per quarter plus a baseline IV Optimization Panel. This saves $91 per quarter compared with $591 in monthly dues.

Executive IV Concierge: $12,000 per year, by application, including unlimited treatments within your clinician-approved care plan.

Clinical eligibility

Membership does not guarantee treatment. All IV hydration, nutrient therapy, injections, and medications require an evaluation and an individualized order from an authorized clinician. Your clinician may decline any request that is not clinically appropriate.

Some options may require additional medical clearance. Availability of specific formulations may change, and compounded medications are not FDA-approved.

Billing and renewal

Monthly memberships bill every month and quarterly memberships bill every three months until cancelled. The Executive IV Concierge membership bills annually.

Included treatments are intended to be used within the applicable membership period. Rollover, transfer, and unused-treatment rules are described in your written membership agreement.

Cancellation

You may cancel according to the notice period stated in your written membership agreement. Cancellation stops future billing; amounts already billed for a completed period are generally non-refundable unless required by law.
